This is an application to voluntary surrender a previously issued Drugs and Alcohol Management Plan Approval.

2- Review:

CAA confirms the request is voluntary and checks for any unresolved audits or pending actions.

3- CAA Decision:

Approval status is marked as cancelled in the CAA system and a formal notice is issued.
