This application is aimed at any accredited Medical Investigation and Diagnostic Facility whose accreditation is approaching expiration and wishes to apply for renewal.

Workflow:
1- Application:
Facility submits an application with all required documents to the CAA.
2- Review:
CAA reviews application form and attached documentation.
3- Inspection:
CAA may conduct an on-site inspection to verify compliance
4- CAA Decision:
Upon satisfactory review and inspection, CAA issues approval for the facility as a Medical Investigation & Diagnostic Facility.
Required documentation:
- Application letter from the facility to CAA
- Copy of MOH Medical Facility Approval/ License
- Copy of the Valid Health Authority License for the Medical Staff (if updated)
- Copy of the CME records for the past two years (for renewal only)
- Copy of Equipment Calibrations
- Copy of Facility Scope of Work (if changed)
- Quality Management System (QMS) Documentation (if updated)
- Copy of Standards Operating Procedure (SOP) (if updated)
- Copy of referral arrangement(s) with other laboratories, advanced imaging centers, etc (if updated)
- Copy of Medical Record Policy (if updated)
- Copy of Medical Waste Contract (if updated)
