This application is aimed at any medical certificate holder who wishes to apply for an amendment to their Medical Certificate.

Workflow:
- Application Submission
- ยท The applicant submits an amendment request through the designated platform, indicating whether the change is related to:
- Bio Data (e.g., name, nationality, date of birth), or
- Certification (e.g., validity extension, removal of limitations, change in fitness conditions).
- Supporting documentation must be attached with the request.

- Initial Review by the CAA
The CAA conducts an initial assessment of the request:

- For Bio Data changes: The CAA verifies the submitted documents. If valid, the amendment is processed administratively.
- For Certification-Related changes: The CAA Medical Assessor evaluates the request and reviews the supporting medical evidence.
- Referral (if required)
- If further evaluation is necessary to support a certification-related change, the CAA Medical Assessor may refer the case to one or more of the following:
- A Designated Medical Examiner (AME) or an Aeromedical Center (AeMC) for clinical reassessment.
- A Medical Investigation Facility for specialized testing or diagnostic review.
- A CAA-Approved Collection Officer or Tester, for implementation of the Drug and Alcohol Management Program (DAMP).
- If further evaluation is necessary to support a certification-related change, the CAA Medical Assessor may refer the case to one or more of the following:
- Final Review and Decision
- Once all assessments or referrals (if any) are complete, the CAA reviews the findings and makes a final decision on the amendment request.
- The Medical Certificate is updated accordingly, and the applicant is notified of the outcome.

- Recent passport-sized photograph (4x4 cm, blue background, front view)
- Copy of passport and visa
- Supporting documentation for the requested amendment (e.g., court order for name change, updated passport, medical reports)
