This application is aimed at any CAA-accredited Medical Investigation and Diagnostic Facility that wishes to apply for an amendment to their existing accreditation.

Workflow:
1- Application:
Facility submits a formal amendment request with all relevant documents to the CAA.
2- Review:
CAA reviews the request and supporting documents.
3- Inspection:
CAA may conduct an on-site inspection based on the nature of the amendment.
4- CAA Decision:
Upon satisfactory review and inspection, CAA issues an amended approval certificate to the facility.

- Application letter from the facility to CAA
- Copy of MOH Medical Facility Approval/ License (if changed)
- Copy of the Valid Health Authority License for the Medical Staff (if updated)
- Copy of Equipment Calibrations (if new or additional equipment is introduced)
- Copy of Facility Scope of Work (if changed)
- Quality Management System (QMS) Documentation (if updated)
- Copy of Standards Operating Procedure (SOP) (if updated)
- Copy of referral arrangement(s) with other laboratories, advanced imaging centers, etc (if updated)
- Copy of Medical Record Policy (if updated)
- Copy of Medical Waste Contract (if updated)
